Fees and Payment
Payment is expected at the time of service. If special payment arrangements are necessary, please discuss this with us.
Insurance
We participate with most major insurance companies including Medicare, Medicaid, Blue Cross Blue Shield, PPOM. Please check with our staff prior to your appointment regarding your coverage. Each patient is responsible for any balance not paid by insurance.
Office Hours
We will be open weekdays and select evenings for your convenience. Office visits are by appointment only. If you cannot keep an appointment, please call us at least 24 hours in advance to reschedule. This will allow us to offer the time to others who may be in need of treatment. If you are a new patient, please arrive a few minutes early in order to complete medical and insurance forms.

We invite you to call the office with any medical concerns or questions. If you have an urgent problem, we will attempt to address it immediately.
